PROPER PROCEDURE ON HOW TO REPORT A WORK-RELATED INJURY  

 

        If an employee becomes injured while on the job they should do the following in this order:

 

  •  Notify YOUR Supervisor Immediately.
 
  • Call InterAid Triage @ 1-800-367-5020 (Unless this is a life-threatening emergency call 911)
 
  • Go to the designated physician that InterAid Triage refers you to go to. (Unless you have a predesignated form on file in the Human Resources office PRIOR to your injury.)
 
  • After you receive treatment, please call the Human Resource office (559) 877-6209 Ext. 2013 to speak with Jessica Fairbanks to receive information on filing a Workers’ Compensation Claim.

 

Note: The district will NOT be responsible for payment of Workers' Compensation benefits for an injury that occurs from your voluntary participation in any off-duty, recreational, social, or athletic activity that is not part of your essential job duties.
